We are a non-profit charity, providing hospice and community services to the people of New Tecumseth and surrounding areas supporting living life fully and dying with dignity. We walk together with families and community members through the journey of illness and grief offering a variety of day programs, counselling, and palliative care. We believe everyone should have access to the right palliative and bereavement care when and where they need it. We are excited to be developing plans to expand our services to the community in the near future.
SCOPE: Reporting to the Nursing Manager, the Intake and Transition Coordinator coordinates the intake, assessment, admission, development of care plans and discharge for clients both in the community and in the residence.
KEY RESPONSIBILITIES:
- Intake, Assessment and Admission/Discharge to residence for End of Life, Caregiver Relief or Painand Symptom Management as well as discharge planning
- End of Life Nursing Care Plan for Residence and Outreach Care Plan Development

DUTIES & RESPONSIBILITIES:
1. Residence Intake and Assessment
- Provide intake and assessment for the Matthews House Hospice (MHH) residence programs including End of Life, Caregiver Relief and Pain & Symptom Management in collaboration with the Care Coordinator from Ontario Health at Home (OH@H)
- Coordinate hospital and home visits with potential clients/residents and their families, as needed
- Educate all stakeholders (family, hospital staff, health care partners, etc) of relevant processes to access Matthews House Hospice care and facilitate a smooth transfer to the residence, when appropriate
- Recommend and provide information on Matthews House Hospice community hospice programs, when appropriate
- Manage all aspects of discharge planning for residents that return home from hospice after a caregiver relief or pain & symptom management admission and, for residents who are admitted for end of life and improve while in hospice. Continue to provide support to residents that return to the community, if appropriate.
2. End of Life Nursing Care Planning for the Residence and Outreach Care Plan Development
- Develop and coordinate client nursing care plans for all admission types in collaboration with the team, client and family
- Provide holistic guidance, education and support to clients and their families on palliative care, MHH services and community resources
- Assess clients for Hospice at Home support and assist with developing care plan for this service
- Develop and maintain collaborative relationships with physicians, pharmacies, home care agencies, hospitals, places of worship, and community resources on behalf of clients
3. Quality of Care and Mentorship
- Mentorship and support of Registered Nurses (RN), Registered Practical Nurses (RPN) and Personal Support Workers to assist them in providing excellent care with regards to pain and symptom management in accordance with the Regulated Health Professions Act, the College of Nurses of Ontario (CNO) Standards and MHH’s mission, values, policies and procedures
- Participation in the hiring and performance development of RN, RPN and PSW staff in collaboration with the Nursing Manager and Integrated Care Team Manager
4. Health & Safety
- Ensure that provided services are compliant with local/provincial standards and regulations regarding both worker and client health & safety in collaboration with the Joint Health & Safety Committee and the Chief Executive Officer.
5. Interdisciplinary Teamwork
- Actively participate in the growth and development of MHH programs, strategic planning and a positive work environment by interdisciplinary collaboration, teamwork
- Support the organization’s accreditation processes and related quality improvement initiatives
- May be called upon to provide coverage in different care settings, including the residential floor, hospice-at-home services, and the Day Program, depending on operational needs
6. Other Duties as Assigned
QUALIFICATIONS & EXPERIENCE
- Registered Nurse with the College of Nurses of Ontario
- Malpractice Insurance as required by the Regulated Health Professions Act
- Certificate in Fundamentals of Palliative Care
- CNA Certification in Hospice Palliative Care Nursing OR are actively pursuing EnhancedFundamentals and the Comprehensive Palliative Education Program (CAPCE)
- Minimum of 3 years (in the past 5) of experience in hospice/palliative care essential with preference given to candidates with palliative and end of life care experience in a community setting
- Undergraduate Degree in Nursing/Nursing Science is considered an asset
- Membership with the Registered Nurses’ Association of Ontario (RNAO) is considered an asset
- A strong and clear understanding of ethics, especially as related to hospice palliative care
- Demonstrated ability to work as part of a respected team with strong communication and self awareness skills
- Demonstrated ability to adapt to changes in patients, partners, priorities and outcomes and committed to continuous learning and improvement
- Innovative and able to “think outside the box” to reduce barriers to accessing hospice programs
- Encourages a positive and collaborative team environment that promotes building relationships
- A system-thinker and able to connect the dots between hospice work and the broader healthcare system
- Strong computer and documentation skills
JOB REQUIREMENTS & WORK ENVIRONMENT:
- Physical ability to carry out nursing duties as required including standing, walking, reaching withhands and arms, stooping, kneeling, crouching, pushing and pulling, lifting/moving up to 25lbs
- Must have a car and valid driver’s license
- On-call support is required for this position on weekends on a rotating basis
- This role requires full-time, onsite presence. Virtual or remote work is not applicable.
