We are a non-profit charity, providing hospice and community services to the people of New Tecumseth and surrounding areas supporting living life fully and dying with dignity. We walk together with families and community members through the journey of illness and grief offering a variety of day programs, counselling, and palliative care. We believe everyone should have access to the right palliative and bereavement care when and where they need it. We are excited to be developing plans to expand our services to the community in the near future.
SCOPE: The Personal Support Worker (PSW) works in collaboration with clients/patients, families, and the Hospice Services team in providing direct, client/patient focused personal care and support in the residence, community and in the Memory Care & Dementia Day Program.
DUTIES & RESPONSIBILITIES:
- Reports to Manager, Integrated Care Team
- Provide personal care and support to palliative and end of life care individuals and their caregivers in community
- Actively participate in all aspects of client care according to the client’s care plan including daily hygiene and activities of daily living, light housekeeping, meal preparation, shopping etc.
- Ensure the psychological, social and spiritual needs of each client are met according to the care plan.
- Work in compliance with the Occupational Health & Safety Act and MHH health & safety rules and policies including infection prevention & control standards/practices
- Report to the Hospice at Home Co-ordinator or Manager, Integrated Care Team regarding any problems or issues that arise with a client
- Promote community awareness of hospice palliative care and the services provided by Matthews House Hospice.
- Attend staff and program meetings and in services, as required
- Document on clients in accordance with Matthews House Hospice clinical documentation policy
- Other duties as assigned including supporting patients in the hospice residence and clients in the day program when required
QUALIFICATIONS & EXPERIENCE:
- Registered with the Health and Supportive Care Providers Oversight Authority is required
- Personal Support Worker Certification or equivalent
- Demonstrates knowledge of hospice palliative care and end of life issues
- Demonstrates comfort in managing, working alongside and supporting volunteers and students.
- Completion of Fundamentals of Hospice Palliative Care is an asset
- Demonstrates ability to work as part of a respectful team
- Demonstrates understanding of ethics, especially as related to hospice palliative care
- Demonstrates excellence in communication, interpersonal, organizational and time-management skills, proficient in MS Office products
- Demonstrates knowledge of relevant community resources

- Physical ability to carry outPSW duties as required (regularly required to stand, walk, reach with hands andarms, stoop, kneel, crouch, push, pull, regularly lift/move up to 35 pounds.
- Excellent interpersonal and listening skills with an ability to workeffectively with others including patients and families
- Utilization of mechanical lifts and other medical equipment to support care provision
- Ability to work independently and with an interdisciplinary team
- Valid driver's license and appropriate vehicle for traveling to and from community visits
- Commitment to continuous learning and professional development
- Computer proficiency with experience working with electronic medicalrecords
- Adhere to the confidentiality policy of Matthews House Hospice
- Excellent attendance record
- Positive attitude with respect for Matthews House Hospice and Matthews House Hospice Foundation mission, vision and values and willingness to work as part of a team
- Clear Criminal Record and Vulnerable Sector check
TRAINING & SCREENING REQUIRMENTS:
- Current CPR and willingness to re-certify, as required
- Advanced training in geriatrics an asset
- Gentle Persuasive Approaches in Dementia Care and/or Positive Approach to Care courses
- Complete annual training as directed by MHH
- Complete MHH Annual Declaration Form
TIME COMMITMENT:
Rotating shifts, days, evenings and nights 7 days a week and holidays
